Vaccinated out of province?
If you received any COVID vaccinations out of province or out of country, you may register those shots with Alberta Health Services' vaccination registry. This will allow any appropriate healthcare provider in Alberta to view this information, and may be used as part of your vaccination certification (passport).

Your information should include:
1. clear copy of your front and back of your vaccination receipt
2. full name as it appears on your Alberta Health card
3. personal health care number
4. date of birth
5. current address
6. phone numbers (in case they need to verify with you)
